Fortavia Logistics LLC operates this website. This page explains what we collect when you use it, why we collect it, and what we do with it. We have tried to write it in plain English rather than legal filler.

What we collect

The only information this website collects is what you type into the driver application form and choose to submit:

• First and last name
• Email address
• Phone number
• Years of OTR experience
• Type of freight you have run
• Whether you hold a hazmat endorsement
• Years with your current or most recent carrier
• Whether you have a clean driving record

If you call or email us instead, we keep whatever you tell us in that conversation.

What we do not collect

This site does not use cookies. It does not run Google Analytics or any other analytics or advertising tracker. We do not build a profile on you, we do not follow you across other websites, and there is no tracking pixel on this page.

Why we collect it

To evaluate your application, contact you about driving opportunities, and answer your questions. That is the only reason. We do not sell your information, rent it, trade it, or hand it to marketing companies.

Who else touches your data

Three third parties are involved in running this site:

• FormSubmit processes the application form and delivers it to our inbox. Your submission passes through their systems on the way to us.
• IONOS hosts this website and our email.
• Google Fonts serves the typefaces on this page. Loading them sends your IP address to Google. We do not receive or store that information.

If you would rather not have your details pass through a form service, call us at (513) 224-2251 instead.

Driving records and background checks

The application asks whether your record is clean. That is your own statement, and submitting the form does not authorize us to pull anything.

If your application moves forward, we will need to verify your CDL and review your motor vehicle record, and depending on the role, run a background check and confirm your status in the FMCSA Drug and Alcohol Clearinghouse. Before any of that happens, we will ask you for separate written authorization and give you the disclosures the law requires. You will know exactly what is being checked before it is checked.

Who we share it with

If we move forward with your application, we may share your information with a carrier we would lease you on to, for the purpose of getting you working. We will tell you which carrier and why before we do it.

We may also disclose information where the law requires it, such as a subpoena, court order, or a DOT or FMCSA request.

How long we keep it

Applications that do not move forward are kept for up to two years, then deleted. If you are hired, your records become part of your driver qualification file, which federal regulations require us to keep for the length of your employment plus three years.

Your choices

Email operations@fortavialogistics.com at any time to ask what we have on file, correct something that is wrong, or ask us to delete your application. We will respond within thirty days. If federal regulation requires us to retain a record, we will tell you which one and why.

Security

This site runs over an encrypted connection and applications go straight to a password-protected business inbox. No system is perfect, and we will not pretend otherwise, but we do not keep your information anywhere it does not need to be.

Children

This site is meant for professional CDL drivers. We do not knowingly collect information from anyone under eighteen.

Changes

If we change how any of this works, we will update this page and the date at the top.
